Can a doctor tell your parents if you smoke?

Im 14 and i smoked for about 4 months. but i quit about 4 or 5 weeks ago so wil he be able to tell? but i from smokin to snuff and i have a physical tomorrow and i just had a dip of snuff. i have only been doin snuff for about a week or two. willl the doctor believe me if i say its sunflower seeds? and if it doesnt, can he tell my parents i do dip snuff? i have my appointment tomorrow. you can see some lines in my inner lip, but not extreme.
