Need help. my idea for a tattoo...ideas to the concept?

I think it would be cool if you had an outline of a head but inside the head have a person like in mental hospital or have a person walking like down a street or something but have it be all surreal with disturbing things and have the person look distraught obviously these would be large pieces of art work but you didn't say what size you were thinking about
